The hospital pays $3,875,000 a year in anesthesia subsidy. Reconciled against the group's own cost stack and collections, the defensible figure is $3,508,170. The gap has been in the contract for three renewal cycles.

Market exposure

● Live production data Wisconsin · period 2026-07

Every figure in this module is queried from the live AOC market set — 271,745 postings across 3,632 employers and 34 months. The facility above is synthetic; the market it sits in is not.

Methodology note. The raw WI CRNA percentile set reads p25 $280,000 / p50 $360,000 / p75 $370,000 (n=18) — inflated by agency postings that advertise contract rates, not salaries. Restricting to direct employer postings gives an average midpoint of $298,421 (n=57); a 12% posted-to-realized haircut puts the employed benchmark at $262,000. Meridian pays 11.1% under it.

Read down the CRNA column and the methodology defends itself. The headline state median says $360,000 and is unusable — it is measuring the locums market. Remove the agencies and Wisconsin lands at $298,421, within 2.6% of the national $290,750. That is the number a hospital competes against, and Meridian is 11.1% below it.

Causal chain CRNA pay 11.1% below the direct-posted market → 3.5 FTE of persistent vacancy → agency backfill at $487,500 effective cost per FTE → a $662,410 annual premium over employed cost, inside a market where seven of every ten open roles are already posted by a recruiter.

The same object graph the scenario runs on, drawn out. Move a lever and the edges it actually touches light up — which is how you can tell that block discipline never reaches cost directly. It only decides how many sites you are allowed to take out.

What you are looking at

The facility is synthetic. The market is real. Meridian Regional does not exist. Its case log, block grid, staffing roster and contract are generated from a documented parameter set. Everything it is measured against — state compensation percentiles, agency saturation, competing employers, ownership structure — is queried from AOC's production market set.

The model is a model, not a spreadsheet. Each lever moves the same object graph a deployed instance uses: Facility → AnesthetizingSite → BlockAllocation → Case → Provider → Contract. Change compensation and vacancy, locum share, group cost and required subsidy all move together, because they are the same objects.

Posted is not realized. Advertised compensation runs ahead of W-2 outcomes, and agency postings run ahead of both. Where a benchmark is used here, the haircut is stated on the panel rather than buried.

Small cells are suppressed. A state-and-role median built on fewer than fifteen salaried postings is not published, and single-employer clusters are excluded. The WI CRNA figure carries its own note for exactly this reason.

Nothing here is a projection. The scenario tool reports what the cost stack does under a stated assumption. It does not forecast, and it deliberately declines to model a saving it cannot attribute to a line.

On your data it takes an extract, not an integration. Block schedule, case counts by room and service, staffing roster, contract terms. No PHI, which is what keeps a first deployment measured in weeks.
